LOSpro Offers FREE Care Coordination Assessments

Care Coordinators are professionals such as RNs, Social Workers or Case Managers who hold valid Licenses in the States in which they are working. A Coordinator can assist you with initial or with short term needs. They can point you in the right direction and help you find the services you are looking for. A Care Coordinator works with LOSpro on a regular basis and generally resides within the territory they manage, thus they are very familiar with services available there. Although the phone consultation is FREE, we do accept DONATIONS HERE if you would like to thank your Care Coordinator.

If after your free phone assessment you find you need further assistance you may choose to hire a Care Coordinator or Geriatric Care Manager (see below) on an extended basis. The costs for these services varies depending on location, qualifications, and actual services provided but usually average $50 per hour. They are billed on a fee for service basis and are not covered by Medicaid, Medicare, or most private insurance policies. The Geriatric Care Managers (GCMs) listed with LOSpro are certified by the governing body of the GCM organization and work independently from LOSpro. GCMs are hired professionals who specialize in assisting patients and their families in meeting their day-to-day and long-term care needs. They are a rapidly growing group of experts in their field who can help families adjust and cope with the challenges of an aging or ill loved one. They also act as liaisons to families who are separated by long
 distances from their elderly loved ones making sure they are managing well, and alerting them to any concerns or problems that may arise. Geriatric Care Managers have extensive knowledge about the services and resources in their communities. The costs for these services varies depending on location, qualifications, and actual services provided. These fees range from $50 to $150 per hour. They are billed on a fee for service basis and are not covered by Medicaid, Medicare, or most private insurance policies.
